Before you Begin Try-Outs!
All potential athletes must have the following forms turned into either their head coach or the bookkeeper.
1. Valid Physical Exam - physicals are good for one year
2. Proof of Medical Insurance - all athletes must have either basic health insurance or purchase health
insurance through our district. Forms to purchase health insurance can be found in the Athletic Director or Bookkeeper offices.
3. Emergency Card

Transportation Fee:
All West HS athletes will be asked to pay $150 per sport for athletic transportation. Athletes who do not pay will not be allowed to participate in that season of sport. Athletes who need financial assistance should submit a Fee Waiver application with supporting financial docments to the bookkeepers office. West HS administration will review the application and athletes will be notified of how much aid they will receive. Waivers must be received prior to the deadline. Winter sports deadline is Friday, November 18. Spring sports deadline is Friday, February 17. Waivers submitted after the deadline will not be reviewed and those atheletes will be asked to pay the full $150.
