1. Valid Physical Exam - physicals are good for one year

2. Proof of Medical Insurance - all athletes must have either basic health insurance or purchase health insurance through our district. Forms to purchase health insurance can be found in the Athletic Director or Bookkeeper offices.

3. Emergency Card - Forms can be found in Handbook and Forms Section.

4. Signature Pages: Athletes must turn in Signature Pages for the Athletic Handbook Acknowledgement & Consent, Steroid Education and Concussion Education prior to beginning practice or tryouts. The Signature Pages can be found in the Handbook and Forms Section.

Transportation Fee:

All West HS athletes will be asked to pay $150 per sport for athletic transportation. Athletes who do not pay will not be allowed to participate in that season of sport. Athletes who need financial assistance should submit a Fee Waiver application with their most recent tax return to the bookkeeper’s office. West HS administration will review the application and athletes will be notified of how much aid they will receive. All Waiver applications must be submitted to the bookkeeper one week prior to the Fee deadline. Waivers submitted after the deadline will not be reviewed and those athletes will be asked to pay the full $150.

Transportation Fee Deadlines are September 1 (Fall Sports), December 1 (Winter Sports), and March 1 (Spring Sports).
